Harlem Residency in Family Medicine
In collaboration with The Mount Sinai Hospital, the Harlem Residency in Family Medicine is sponsored by the Institute for Family Health and is based in historic East Harlem. The program educates family physicians in the provision of longitudinal, preventive, comprehensive and accessible family-centered care to underserved, medically complex patients.

Family Medicine Resident General Summary:

The Family Medicine Resident is responsible for completing the educational requirements set by the family medicine resident program, as well as the Accreditation Council for Graduate Medical Education (ACGME) and the American Board of Family Medicine (ABFM) or American Board of Osteopathic Family Medicine (ABOFM). Under the direction of the Residency Program Director, the Designated Institutional Official (DIO) and the residency program faculty, the Family Medicine Resident will participate in direct patient care in both inpatient and outpatient settings, educational activities, such as didactic lectures, and scholarly activities, such as quality improvement and research.

Essential Job Functions:

The job duties listed are essential functions of the position. However, other duties may be assigned, and may also be considered essential functions of the position. The caregiver must be sufficiently fluent in the English language to satisfactorily perform the essential functions of the position. The degree of fluency required will vary depending upon the nature of the position. Caregivers are expected to honor the Mission, Values, Vision and Promise and adhere to the Code of Conduct, policies and standards of their organization. For direct patient care roles: Performs and maintains currency of essential competencies as required by specific area of hire and populations served.

Comply with the responsibilities set forth in the โ€œGraduate Medical Education Training Agreement for the Family Medicine Residency Programโ€, which include providing required documentation to the program to verify eligibility to participate in the training program. Read and comply with the sponsoring institutionโ€™s Graduate Medical Education (GME) policies.

  • Provide, under supervision of the attending faculty and program director, direct patient care in both the hospital, primary residency clinic, and various other settings.

โ€ข Comply with the ACGME Duty Hour requirements, including the honest, accurate and timely reporting of Duty Hours . โ€ข Attend and participate in New Resident Orientation during the first year of the program. โ€ข Attend and participate in orientations for specialty rotations as required by the program. โ€ข Complete annual employee trainings required by the hospital, including Health Stream modules. โ€ข Complete all medical records in accordance with the Program policies, as well as the policies of the participating sites where the Resident receives training.

  • Complete all patient care responsibilities in a timely, accurate manner as required by the participating sites where the

Resident receives training.

  • Participate in the ACGME annual resident survey, annual program survey, faculty evaluations, and any other evaluations

required by the Program.

  • Participate in activities assigned and required by the program, including patient care rotations, didactics or other

educational activities, research activities, and local, national or regional conferences.

  • Participate in activities that assess the Residentโ€™s competence, including evaluation sessions, direct observation by

faculty, In-Training Examinations, and written assessments.

  • Notify the program and office of GME immediately and in writing in the event of any changes to the Residentโ€™s ability to

continue training.

Licenses and Certifications:

Eligible for a training license to practice medicine by the California Medical Board.

  • Obtain a postgraduate training authorization letter (PTAL) or postgraduate training license (PTL) as required by the

California Board of Medicine by December 2020.

  • Obtain an NPI number when eligible within 120 days of employment.
  • Obtain DEA license registration when eligible by December 2020.
  • Basic Life Support (BLS) certification.
  • Within 120 days of employment, obtain and maintain certification in:
  • Advanced Cardiac Life Support (ACLS).
  • Pediatric Advanced Life Support (PALS).
  • Neonatal Resuscitation Program (NRP).
  • Advanced Life Support in Obstetrics (ALSO).
  • Advanced Trauma Life Support (ATLS).
