The IPAP students complete their first 16 months of education at Fort Sam Houston, Texas. During Phase 1, students complete a challenging curriculum, including 101 written examinations, numerous lab practical assessments, and over 2,640 contact hours of expert instruction. A master’s thesis paper is due prior to Phase 1 completion. The curricula is developed and implemented in accordance with the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA), the NCCPA blueprint, and service requirements. Competencies stressed are medical knowledge, interpersonal and communication skills, patient care, professionalism, practice-based learning and improvement, and systems-based practice.

Phase 2 is completed at various military installations throughout the United States, with Navy students completing clinical rotations in San Diego. These rotations include: surgery, orthopedics, dermatology, obstetrics/gynecology, psychiatry, internal medicine, ENT, pediatrics, emergency medicine, family medicine, and clinical electives such as radiology, trauma, or anesthesiology. Additionally, there are weekly journal club meetings with student presentations, and small group activities.

The IPAP is accredited via ARC-PA. All indicators point to a highly successful multi-service program graduating outstanding Officer Clinicians every four months. Feedback from the respective services continues to be positive in nature. The IPAP is also highly regarded among other PA programs, receiving a US News and World Reports ranking of # 11 out of all programs in the United States. For perspective, there were 196 accredited programs in the U.S. at the time of this ranking. The IPAP also continues to be designated as the largest PA program in the world.

IPAP graduates serve in Navy MTFs, outpatient clinics, aircraft carriers, with the Marines, CONUS and OCONUS. Most Navy PAs provide primary medical care in a family medicine/outpatient medicine environment when not deployed. This can be in a military medicine clinic, urgent care clinic, internal medicine, pediatrics, etc. There are also positions available at the White House, BUMED, NPC, and instructor duty at the IPAP and SWMI San Diego. Many Navy PAs go on to orthopedic, general surgery or emergency medicine specialty training or pursue other graduate studies, either via DUINS or other programs. There are numerous opportunities for personal and professional growth as a Navy PA, and more leadership and clinical opportunities open up every year.

The Interservice Physician Assistant Program (IPAP) has been in operation since May 1996. Prior to that time, the Navy, Army, and Air Force and ran their own similar, but separate programs. The IPAP is part of the Graduate School, AMEDDC&S Health Readiness Center of Excellence, Joint Base San Antonio, Fort Sam Houston, Texas. The IPAP has a university affiliation contract with the University of Nebraska Medical Center (UNMC). The UNMC has agreed to provide administrative support to the IPAP and award academic credit to students completing the IPAP requirements. Students who successfully complete the IPAP are granted a Master of Physician Assistant Studies (MPAS) degree from the University of Nebraska Medical Center.

The Department of Defense and Department of Homeland Security have entrusted the IPAP with their most outstanding service members. The IPAP faculty and staff continue to enhance the educational mission in producing the finest military PAs and officers. All IPAP graduates meet National Commission on the Certification of Physician Assistants (NCCPA) eligibility for certification. As a result of the Phase 1 (didactic) curriculum and Phase 2 (clinical) experience, students have a 99% pass rate with a mean PANCE score consistently above the national average. Graduate success is directly related to an educational process that continuously incorporates advances in medical information, instructional methods, and technology.

How to Become a Doctor in the Navy: 11 Steps (with Pictures)

I received a few e-mails asking for help figuring out physician pay in the Navy, and this is a long overdue blog post. In the spreadsheet below is the pay info for the various stages as you move throughout your Navy Medical Corps Career. I’m making a few assumptions:

These are FY20 pay numbers (since the FY21 pay plan is not out yet).
You promote at the normal times (O4 at 6 years, O5 at 12 years, and O6 at 18 years).
Basic Allowance for Housing is with dependents in San Diego. You can personalize this here.
The specialty is Emergency Medicine. You can look at the different amounts for other specialties here.
You pass your boards and become board-certified after residency.
For those who don’t want to look at the spreadsheet, here are the bottom line annual salaries:

New O3 intern – $95,976
O3 GMO – $121,803
Mid-grade O3 EM Resident – $120,348
New O4 EM Attending – $180,249
O5 EM Attending on a 6-Year Retention Bonus – $264,665
O6 EM Attending on a 6-Year Retention Bonus – $287,878

When you start talking compensation it is often quickly compared to civilian pay.

However you need to look at your take home income.
That is your paycheck after taxes. Your taxable income is VERY low compared to it all being taxed as a civilian.

I believe you’ve posted GAO-20-165 elsewhere but it may be worth it’s own treatment. While the report is often referenced for its comparison of maximum military pay by rank to civilian pay percentiles (Fig. 4, pp. 22), that comparison does little service to the body of the report. There is a wealth of useful information in there beyond pays including rigorous evaluations of compensation value vs civilian practice; physician recruitment; physician retention; obligated service; and future directions for the medical corps. https://www.gao.gov/assets/710/703877.pdf

I’m a little late to the party, but first off this website has been a gold mine! Thank you Dr. Schofer. I’m having difficulty fully navigating through this site, but is there a forum discussing or calculating the compensation between EM civilian pay and EM military if I were stick out the 20 years? With taxes, retention bonuses, and my incomplete understanding of my complete benefits after the 20 years it’s difficult for me to crunch my own numbers. I’d appreciate any references if anybody has it.

A Lieutenant Commander is the fourth rank among officers in the USN. You usually refer to them as “Lieutenant Commander”, usually not “Commander”, and never “Lieutenant” — that would be an instant demotion!

“Billets” (jobs) held by LCDRs vary by community. In the Surface Warfare community, a LCDR may be the Commanding Officer of a smaller ship such as a Minesweeper. On larger ships such as Submarines or Destroyers, a LCDR may be either a senior Department Head or the Executive Officer (second-in-command). LCDRs are shown the due respect of a an officer prospecting to become a Commanding Officer of a larger ship or unit.

In the Direct Commission Officer community, O-4s are professionals with equivalent years of experience. For example, a surgeon with ten years of experience who joins the Navy may receive a commission and immediately receive the rank of LCDR.

What is a Lieutenant Commander’s Paygrade?
A Lieutenant Commander is considered a Junior Officer, with a paygrade of O-4. The civilian equivalent of this military rank is roughly GS-12 under the federal government’s General Schedule payscale.
